The PopHealth Learning Center (“Learning Center”) is a California Social Purpose Corporation (SPC) committed to transforming how health care is delivered and experienced across California, with a focus on low-income communities. The Learning Center serves as the Program Office for a statewide Medi-Cal focused practice initiative, the Equity and Practice Transformation (EPT) program, a $140 million-dollar investment in primary care transformation funded by the California Department of Health Care Services (DHCS). The Learning Center also works with several key partners throughout California on aligned programs to build Population Health Management (PHM) capabilities and improve health outcomes for Californians. We serve as a strategic partner to organizations and institutions committed to advancing health equity, with all efforts grounded in a shared goal: supporting a more coordinated and person-centered healthcare system that strengthens primary care and advances equitable access across California’s safety net.
The Learning Center is highly invested in making data enabled care a reality in the safety net of California. Primary care practices serving low-income communities have historically had disparate access to technology, data, and innovations like Artificial Intelligence. The Learning Center incorporates data enablement as a foundational element to all practice improvement work. Recently, The Learning Center has advanced its data enablement portfolio by partnering with Kaiser Permanente in the Population Health Management Initiative (PHMI) to scale deployment of the single instance PHMI Innovaccer platform for Community Health Centers and independent practices across the state. The PHMI platform enables secure, standardized access to clinical, claims, and social needs data to drive whole-person care coordination across California. Through these initiatives, The Learning Center is building an integrated data ecosystem and advancing scalable solutions that strengthen quality, accountability, and performance improvement statewide.
The Position
The Senior Director of products and solutions serves as the Learning Center’s senior product leader, responsible for defining the statewide product vision and driving adoption of population health management products or solutions as well as exploring the development of other high value population health tools. This role ensures the technology strategy advances The Learning Center’s goals for equitable, high-performing population health management across primary care serving low-income communities.
As the senior operational leader for the statewide product and solutions roadmap for the PHMI Innovaccer platform, the senior director oversees product and solutions strategy, platform lifecycle direction, and cross-organizational coordination. The role partners closely with the Chief Health Informatics Officer (CHIO) to ensure product decisions are grounded in clinical workflows, informatics best practices, and value-based care priorities.
This role will be required to be in office at our location in Oakland,Ca for at least 1-2 days per month.
Applications will be evaluated on a rolling basis. California residents are strongly preferred.

- Direct oversight and accountability for the design and implementation of the Learning Center’s product strategy, and other Learning Center population health related programs and products.
- Define and maintain statewide products and solutions roadmap for the PHMI Innovaccer platform and other solutions aligned with the Learning Center’s equity, quality, and transformation goals.
- Ensure all Learning Center product development and direction enables scalable practice transformation, value-based care delivery, and data-driven population health operations.
- Supervise and strategically grow the product and implementation teams to effectively deliver on current organizational goals; continue to build the Learning Center’s portfolio of technology and data enablement work.
Statewide Scaling of PHMI population health management solutions
- Oversee the product lifecycle and guide major platform enhancements ensuring unified execution across practices and partners.
- Lead statewide governance for product decision-making, standards, and performance expectations for shared instances of the PHMI platform.
- Monitor platform performance, user adoption, and alignment with program outcomes, making recommendations for continuous improvement.
- Lead the Learning Center’s work to define and implement successful solutions across various programs, products, and evaluation activities
- Direct the ongoing assessment, design, and implementation of products and solutions that will support the organization's current needs and scale to support growth and new bodies of work.
- Experience evaluating, selecting, and operationalizing healthcare technology solutions and vendor partnerships in complex healthcare environments.
- Experience leading large-scale implementation, adoption, and change management efforts across diverse healthcare organizations or practice environments.
- Serve as the senior point of alignment between the Learning Center, practice sites, payers, KP, and vendor(s) regarding product needs and statewide consistency.
- Ensure the product experience supports real-world clinical and operational workflows across diverse practice environments.
- Establish mechanisms for structured input, feedback, and user research to inform ongoing product evolution.
- Strong understanding of population health operations, care management workflows, quality improvement programs, and value-based care transformation initiatives.
- Work closely with Learning Center operational, programmatic and finance teams to ensure product strategies are aligned and supporting broader organizational and programmatic goals.
- Collaborate with key funders, stakeholders and the Department of Health Care Services to ensure the product strategy is integrated with broader statewide priorities.
- Partner with the CHIO to ensure platform capabilities reinforce clinical workflows, data use needs, and quality improvement strategies.
- Coordinate with vendors such as Innovaccer or other platforms on release planning, risk management, and alignment with statewide implementation objectives.

The ideal candidate will be a mission-driven leader with deep expertise. They will bring a collaborative spirit and a commitment to equity, along with the following qualifications:
- Bachelor's degree and master's or advance degree in healthcare administration, information systems, public health, or a related field; clinical background or experience is preferred
- Minimum of 8-12 years of progressive experience in product management, digital health solutions, or healthcare technology implementation, ideally within population health or care management environments.
- Demonstrated success leading product strategy, development, and lifecycle management for healthcare or population health platforms (Innovaccer experience strongly preferred).
- Strong understanding of clinical workflows, interoperability standards (FHIR, HL7, CCD), and value-based care operations.
- Experience coordinating cross-functional teams and managing relationships across technology vendors, clinical practices, and program leadership.
- Skilled in translating user needs into actionable product requirements and measurable outcomes.
- Experience evaluating, implementing, or scaling digital health, AI-enabled, or workflow transformation solutions that demonstrate measurable improvements in clinical outcomes, operational efficiency, provider experience, or care delivery processes.
- Proficiency in project management, data analysis, and stakeholder communication.
- Commitment to advancing health equity, digital transformation, and practice improvement for historically underserved populations.
- Certifications such as PMP, CSM, or Lean Six Sigma plus.
