The Learning Center is focused on strengthening the infrastructure required to advance equity-focused transformation in primary care and population health. This includes designing and managing large-scale practice transformation initiatives, delivering technical assistance through virtual learning environments and peer-based engagement, and supporting alignment across systems, funders, and care delivery partners. We serve as a strategic partner to organizations and institutions committed to advancing health equity, with all efforts grounded in a shared goal: building a more just, coordinated, and person-centered healthcare system that better serves communities historically marginalized by existing models of care.
The Position
The Improvement Advisor at the Learning Center will lead the implementation of a three-year engagement with a California-based Managed Care Plan focused on advancing primary care transformation and population health management in their provider network. Reporting to the Director of Programs and working with a subcontracted partner, the Improvement Advisor will manage the project’s execution and continuous improvement. In addition to managing day-to-day operations and shaping overall coaching strategy, the Improvement Advisor will directly deliver 1:1 coaching to a subset of primary care practices. This position combines strategic leadership with hands-on support to drive equitable health outcomes through individualized coaching, collaboration, and peer learning across a diverse network of safety net providers.
The ideal candidate will have experience supporting large-scale primary care transformation efforts through hands-on coaching, facilitation, and improvement strategy. They will be skilled at delivering 1:1 coaching to primary care practices, guiding teams through care redesign initiatives such as team-based care, panel management, and population health improvement. The candidate should have a strong track record of applying change management and quality improvement (QI) methodologies—such as the Model for Improvement or Lean principles—to inform program design, guide implementation, and drive continuous learning. Experience engaging diverse stakeholders, including clinical teams and leadership, is essential, as is the ability to use electronic health records (EHRs) and population health management (PHM) tools to support tech enabled workflows, data-driven decision-making and sustainable practice change.

Compensation & Benefits

Responsibilities include:
Coaching and Practice Support
- Provide direct, individualized coaching to primary care practices, guiding them through primary care redesign efforts—including team-based care, panel management, and population health strategies—to maximize performance in selected measures and pay-for-performance programs.
- Adapt and enhance the existing coaching strategy, including the development of updated training materials, tools, monitoring approaches, and resources to ensure ongoing effectiveness across PHLC offerings.
- Conduct onsite, in-person PhmCAT Assessments (Population Health Management Capabilities Assessment Tool) to evaluate practice capabilities and identify opportunities for improvement.
- Design and implement peer learning sessions and collaborative knowledge-sharing experiences to foster a culture of continuous improvement among safety net providers.
Program and Project Management
- Oversee the day-to-day implementation of the three-year engagement, ensuring successful delivery of coaching, assessments, and learning activities, while managing timelines and deliverables.
- Collaborate with subcontractor organizations to ensure alignment in coaching delivery and best practice implementation.
- Track project progress, including coaching outcomes, assessment results, and overall improvements in practice performance. Provide regular updates to stakeholders and adjust strategies as needed.
Stakeholder Engagement and Communication
- Act as the key liaison for the funder, attending regular meetings to provide project updates, address any concerns, and ensure alignment with the funder's expectations and goals.
- Develop data-driven analyses and presentation materials to communicate project progress, impact, and lessons learned to funders, internal teams, and external stakeholders.
- Foster a collaborative and inclusive environment that supports professional growth and development.
- Promote a culture of continuous improvement by identifying opportunities to enhance financial processes, systems, and team capacity.
- Provide training and guidance to teams to build organizational capacity and ensure alignment with organizational goals.
- Act as a cultural steward, ensuring the Learning Center’s values of equity, inclusion, and collaboration are reflected in the finance team’s work and interactions.
Preferred Skills and Qualifications Including:
The ideal candidate will be a mission-driven leader with deep expertise. They will bring a collaborative spirit and a commitment to equity, along with the following qualifications:
- 5+ years’ experience in direct practice coaching; 10+ years’ experience in program development, management and delivery in healthcare systems
- Proven ability to deliver targeted, effective 1:1 coaching to primary care teams from different practice types (e.g., health centers and independent practices), guiding them through care redesign and quality improvement efforts.
- Expertise in QI methodologies such as the Model for Improvement, Lean, and Six Sigma, applied in the context of primary care transformation and population health management.
- Strong organizational and project management skills to oversee a large-scale initiative, including managing timelines, deliverables, and resources.
- Ability to engage with diverse stakeholders, including primary care clinicians, practice administrators, and leadership, to foster collaboration and drive practice improvement.
- Strong facilitation skills for delivering training, workshops, and learning sessions to enhance team capabilities in primary care redesign.
- Demonstrated experience leading or supporting primary care redesign efforts—such as improving performance on quality measures, addressing population health needs, and implementing data-informed workflows—within Medi-Cal and Medicare Advantage contexts.
- Exceptional verbal and written communication skills, including slide development skills
- Intellectual curiosity, growth mindset, and tenacity to get the job done
